NDIS Care Coordinator and Lead Counsellor – Australia Wide

Divina, our esteemed Care Coordinator, holding a Master’s in Counselling from the University of Queensland and recognised as a Level 3 Counsellor by the Australian Counselling Association, seamlessly integrates her skills into the fabric of our clinic.

As a Care Coordinator, Divina plays a pivotal role in providing ongoing support to the participants of our clinic, ensuring safety and progression of care is at the forefront of mind always.

As a Care Coordinator, Divina conducts regular healthcare and safety assessments, specifically formulated to review participants’ key areas of need. Beyond assessments, she actively liaises with key stakeholders, including carers, parents, and all linked Nourished Not Deprived practitioners. Her focus is on identifying and addressing specific areas of need for each participant, ensuring a targeted and effective approach to care coordination.

In her dual capacity as a Level 3 Counsellor, Divina submits monthly to quarterly treatment summaries to Nourished therapists, facilitating collaborative care efforts. She proactively addresses barriers to therapy engagement, from IT literacy to other obstacles, ensuring that participants receive the support they require.

Divina’s role extends to the organisation and management of stakeholder meetings, where discussions and action points are meticulously logged for efficient follow-up. She takes the lead in developing healthcare service plans and recommendations for additional assistance to all involved in the care of each participant, contributing to a strategic approach to care.

Within our company, Divina’s significance is underscored by her ability to align capacity-building supports with the specific needs of participants. Her role extends to monitoring and reporting on the progression of care, emphasising a commitment to transparency and accountability.

Overall, Divina utilises her trained counselling education and experience within the NDIS and healthcare sector, to provide a high quality of care to the participants of Nourished Not Deprived.
