Please carefully review the following details, including the purpose of this form, the nature of telehealth, what it entails, potential benefits, risks, privacy measures, and the meaning of informed consent.

Purpose of This Form

The purpose of this Consent Form is to:

  • Inform you about your appointments with Nourished Not Deprived.
  • Explain the logistics of telehealth-based therapy sessions.
  • Request your informed consent to participate in telehealth consultations, which are offered at your discretion.

What Is Telehealth?

Telehealth involves the use of telecommunication techniques to provide therapy services remotely, rather than through in-person meetings. Telehealth sessions are typically administered through video-conferencing but may also utilize other communication formats, such as telephone or email.

Telehealth Consultation Overview

During a telehealth consultation, your therapist will:

  • Assess your needs and therapy-related goals.
  • Offer information and recommendations, as appropriate.
  • Provide details on how telehealth will be tailored to your unique circumstances.

By entering into our services, you acknowledge and accept the following:

  • You are responsible for the costs associated with setting up the necessary technology for accessing telehealth services, including an appropriate device (e.g., smartphone, laptop, iPad, computer) with a camera, microphone, and speakers, as well as a reliable internet connection.
  • Nourished Not Deprived Pty Ltd will cover the costs of calls to you and the platform used for telehealth services.
  • You are required to access a quiet and private space for telehealth consultations.
  • You may include a support person in your telehealth session, similar to a face-to-face consultation.
  • Video or audio recording of the session is not permitted without your therapist’s permission.
  • Telehealth involves the transmission of medical and mental health information over the internet or telephone connection.
  • Telehealth sessions serve the same purpose as in-person psychological treatment but may be experienced differently due to technology use.

Risks of Telehealth

Despite our efforts to maintain secure end-to-end encryption for telehealth conferencing, there are potential risks, including:

  • Technical failures that could disrupt or distort telehealth sessions.
  • The possibility of unauthorised access to or interruption of telehealth sessions.
  • The risk of sessions being overheard by others, necessitating a private, well-lit, and distraction-free location.
  • Your agreement to adhere to the telehealth service’s terms of service.

Potential Benefits of Telehealth

Telehealth may offer the following advantages:

  • Improved access to therapy services.
  • Reduced travel requirements.
  • Lower exposure to infectious diseases.

Will My Privacy Be Protected?

Nourished Not Deprived Pty Ltd is bound by the Privacy Act 1988, ensuring the collection, use, and disclosure of personal information comply with legal obligations, including those related to telehealth.

Your therapist must uphold confidentiality and privacy standards during sessions and in the creation, storage, and transmission of records. While audio and video recordings may be made to support the therapist’s work, your therapist will seek your consent before recording.

Details regarding the management of your personal information, access to it, and avenues for lodging concerns or complaints must be provided to you upon request.

However, please note that email communication is not secure for transmitting personal information.

Telehealth, including video-conferencing, may expose you to privacy and digital security risks, and complete privacy and security cannot be guaranteed. You can mitigate these risks by using a secure internet connection, conducting sessions from a private location, and utilising secure communication channels.

Informed Consent

Informed consent entails several essential principles:

  • You must receive relevant information, so please feel free to ask your therapist any questions about telehealth and the services offered.
  • You have the right to comprehend the provided information, and your therapist will clarify any uncertainties.
  • You have the freedom to choose; if you disagree with telehealth, you can refuse participation or agree to specific activities within sessions.
  • You can discontinue telehealth at any time, even during a session.
  • You can provide consent verbally or in writing, with verbal agreements documented by your therapist.
  • You may inquire about alternatives to telehealth, as your therapist will explore potential options with you.

Cancellation & Rescheduling Fee (Less Than 24 Hours’ Notice)

Nourished Not Deprived practitioners require a 50% non-refundable deposit upon booking an appointment. Cancellations or no-shows with less than 24 hours’ notice will incur a cancellation fee equal to the full consultation fee. Rescheduled appointments outside the 24-hour notice window will result in a fee equal to the deposited amount, which can be applied to your next booking.

By entering into our services, you confirm your consent to receive therapy services via telehealth through Nourished Not Deprived, acknowledge our Cancellation and Rescheduling Policies, understand the associated risks mentioned in our Terms of Use, Privacy Policy and Consent notices; and recognise your right to refuse services at any time.
